Committee on Integrated Care

Description

A committee within a professional member organization that advises on policy and education to improve psychiatric care access through enhanced coordination and integrated care models. It focuses on financing mechanisms and advancing models integrating behavioral health with general medical care. The Committee collaborates with other APA committees on technology, regulatory issues, and best practices to support integrated care and bidirectional integration.

Cornerstone Whole Healthcare Organization (C-WHO)

Description

A private non-profit organization that supports clinics and payors in implementing and optimizing behavioral health integration. Their team provides hands-on technical assistance, ensuring a seamless transition while enhancing patient care and operational efficiency.

Department of Psychiatry at Michigan Medicine (PRISM)

Description

Provides individualized consultation, training, and implementation support on the Collaborative Care Model (CoCM) to health systems, provider organizations, and payors. Implementation support includes workflow modification, adoption of population-health approaches, sustainability planning, evaluation support, and fidelity monitoring. Team and role-specific clinical training is provided for primary care providers, psychiatric consultants, and behavioral health care managers.

Division of Behavioral Health | Office of Clinical and Preventive Service

Description

A division of Indian Health Service (IHS) which works in partnership with Tribes, Tribal organizations, and Urban Indian health organizations to coordinate national efforts to share knowledge and build capacity through the development and implementation of evidence/practice based and cultural-based practices in Indian Country.

Eugene S. Farley, Jr. Health Policy Center

Description

A team of primary care and behavioral health providers, public health and policy professionals, scientists, educators, students, and fellows focused on health policy research, translation, and implementation to promote whole health for all.

IBH (Innovation in Behavioral Health) Model

Description

A state-based model that leverages patients’ relationships with specialty behavioral health practices to provide whole-person, integrated care that better addresses their behavioral, mental, and physical health. Institute for Integrated Behavioral Health

Description

An institute that brings best-practice team-based care to ETSU Quillen College of Medicine Clinics. Established in 2020, the Institute for Integrated behavioral health into primary care-based residency training clinics within Pediatrics, Internal Medicine, Family Medicine and OB/GYN and expanded behavioral health access in Nursing and high needs community sites.

Integrated Behavioral Health (IBH) at Mayo Clinic

Description

A division of the Department of Psychiatry and Psychology with staff located in primary care clinics across the enterprise. IBH offers time-limited, evidence-based psychotherapy (individual and group), medication consultation and management, depression care coordination, and social service resources to employees and their dependents who receive their primary care, family medicine, and pediatric services at Mayo Clinic.

Lifeline for Moms

Description

A program that promotes perinatal mental health by developing, implementing, evaluating, and scaling approaches for addressing mental health and substance use disorders. Lifeline for Moms: 1) develops and test population-based perinatal mental health programs and related trainings and tools, 2) provides consulting services to facilitate the implementation of mental health into perinatal, obstetric, and pediatric settings, and 3) convenes the National Network of Perinatal Psychiatry Access Programs.

MAHEC Rural Health Initiative

Description

Addresses the region’s rural healthcare provider shortage through interprofessional health sciences education, collaboration with rural communities, and the development of innovative sustainable models of rural healthcare to improve health across Western North Carolina, including a Rural Integrated Behavioral Health Psychology Fellowship.
