Clinical Guideline for the Use of Chronic Opioid Therapy in Chronic Noncancer Pain (PDF - 2.0 MB)
Details evidence-based clinical practice guidelines for use of chronic opioid therapy in adults with chronic noncancer pain.
This collection of tools and resources is for providers, staff, and patients who offer or use services to address substance use, and other interested stakeholders. This collection was originally established following an environmental scan on implementing medication-assisted treatment (MAT) for opioid use disorder (OUD) in rural primary care. (See PDFs of Volume 1 (PDF - 609 KB) and Volume 2 (PDF - 1.3 MB) of that scan). Items have been continuously added to this collection since then, and the collection has expanded to cover substance use more broadly, rather than just MAT for OUD.

Predicts aberrant medication-related behaviors among persons with chronic pain and complements current risk assessment practices to improve a clinician's ability to assess a patient's risk for opioid misuse.
Uses a 17-item self-assessment to identify patients with chronic pain taking opioids who have indicators of current aberrant drug-related behaviors.
Provides guidelines related to opioid dependence that aim to: reduce global barriers to effective treatment; contribute to the development of evidence-based, ethical treatment policies; contribute to improvement of the quality of pharmacological treatment; and facilitate implementation of effective treatment policies and programs.
Assesses the intensity of pain and the level of interference in normal daily life using a three-item scale.
Short guide for a safety planning implementation
Provides recommendations on how to properly use and prescribe opioid medication for both acute and chronic pain.
Provides a summary of recommendations on how to assess and manage chronic pain. Includes detailed tools, such as an assessment algorithm, management algorithm, personal care plan for chronic pain, and the DIRE Score, which predicts patient suitability for long-term opioid analgesic treatment.
Patient safety plan template to fill out with the patient.
