NACHC understands the burden of reporting and complying with the varying terms in value-based contracts. Through this strategy, they hope to streamline reporting burdens for CHCs, incentivize high-value care, and ultimately contribute to improved primary care outcomes.
This session will present the set of quality measures NACHC has identified to put forth in their measure alignment strategy. Come ready to provide feedback on strengths, drawbacks, or any concerns they should be aware of regarding measures they anticipate coalescing around. They welcome everyone to this listening session to provide their opinion; participants do not need to be a “value-based care expert,” just someone passionate about advancing quality measurement alignment!
