The Collaborative Care Model (CoCM) has five core principles: Patient-Centered Team Care, Population-Based Care, Measurement-Based Treatment to Target, Evidence-Based Care and Accountability for Outcomes. CoCM creates opportunities for primary care teams to address behavioral health needs with effective, evidence-based strategies. This webinar, hosted by the AIMS Center, will explore practical, relationship-centered strategies for strengthening provider buy-in and improving communication. Participants will learn how to collaborate with/as a part of medical teams in a way that supports clinical outcomes, efficiency, and shared ownership of patient care.
At the end of this session, participants should be able to:
- Identify key factors that influence medical provider engagement in CoCM.
- Describe effective communication approaches that support strong partnerships for CoCM.
- Recognize relational practices and strategies that enhance buy-in and build trust.
