Peer Support for SMI During Care Transitions

Join the National Council for Mental Wellbeing's Serious Mental Illness Training and Technical Assistance Center (SMI TTAC) for a webinar focused on peer support during care transitions for those experiencing serious mental illness (SMI). 

The webinar will provide a specialized focus on how care transitions (such as inpatient hospitalization/residential settings to outpatient treatment or community) can utilize the peer workforce to increase positive outcomes for those experiencing SMI. Content will include presentation examples from states/providers who’ve successfully integrated peers into care transitions.  

This webinar is intended for Behavioral Health and Substance Use Administrators, Care Providers, Case Managers, Peer Recovery Workers, and other program staff who have direct contact with individuals with SMI, but any interested individuals are welcome to attend!
