In this presentation hosted by the Case Management Society of America, the participant will be able to define the change management models related to various organizational care delivery systems to discover the potential for improvements in health equity, care, outcomes, and satisfaction. Using the Professional Science (ProSci) Awareness, Desire, Knowledge, Ability, and Reinforcement (ADKAR) model as a practical application of improving standards of case management practice. The presentation shares the outcomes related to an innovative leadership skill-based training, The Leadership Academy. This module-based training aligns organizational change management with the Care Coordination and Integrated Case Management initiative in the Veterans Health Administration. Key strategies are shared that involve Communication, Collaboration, and Coordination within the healthcare systems to deploy a seamless transition of care. Using employee and manager engagement principles to strengthen the approaches related to service delivery within organizational settings that provide care coordination and case management services. The future of case management requires leadership skills with an awareness of how to communicate changes and the benefits to the client. This presentation will provide key strategies related to communication of organizational changes, managing resistance, and obtaining key stakeholder buy-in.
