State-Based Healthcare Extension Cooperatives - NOFO Released by AHRQ

On September 9, 2024, AHRQ announced three funding opportunities to support state-based solutions to accelerate health care improvement. A Notice of Funding Opportunity (NOFO) for the first of three, State-based Healthcare Extension Cooperatives to Accelerate Implementation of Actionable Knowledge into Practice (U19), is now published. AHRQ will award up to 15 grants to establish and support state-based Healthcare Extension Cooperatives, over five years. To support the first initiative focused on behavioral health, Cooperatives will:

  • Engage key stakeholders such as Medicaid; Medicaid managed care organizations; clinicians and staff from safety-net health care delivery organizations; and patients, families, and caregivers who receive care from safety-net health care delivery organizations and are members of uninsured and/or medically underserved populations;
  • Work with health care policy, payment, community, care delivery, and research organizations to build their capacity to implement patient-centered, evidence-based health care delivery improvements and support ongoing learning through training and other tools;
  • Conduct evaluations of the Cooperative's activities; and
  • Provide the support structure to ensure these activities are integrated and aligned.

The total costs (direct and indirect) for a project awarded under this NOFO will not exceed $6.25 million in any given year and $25 million for the entire project period.  Applicants may propose different total annual budgets based on their proposed approach.

The application for this NOFO is due by December 12, 2024. In coming weeks, AHRQ plans to publish NOFOs for the next two funding opportunities: a National Coordinating Center and a National Evaluation Center to support the Healthcare Extension Cooperatives.
