This collection of tools and resources is for providers, staff, and patients who offer or use services to address substance use, and other interested stakeholders. This collection was originally established following an environmental scan on implementing medication-assisted treatment (MAT) for opioid use disorder (OUD) in rural primary care. (See PDFs of Volume 1 (PDF - 609 KB) and Volume 2 (PDF - 1.3 MB) of that scan). Items have been continuously added to this collection since then, and the collection has expanded to cover substance use more broadly, rather than just MAT for OUD.
Assesses symptoms of anxiety using a 7-item questionnaire that asks whether the patient has experienced anxiety-related problems in the previous 2 weeks and the impact they have had on the individual's life.
Assesses the risk of opioid abuse and suitability of candidates for long-term opioid therapy in primary care.
Assesses risk for opioid abuse among individuals prescribed opioids for treatment of chronic pain with a brief self-report screening tool designed for use with adult patients in primary care settings. Patients categorized as high risk are at increased likelihood of future abusive drug-related behavior.
Provides guidance on the use of buprenorphine among a number of topics, including physiology and pharmacology of opioids, opioid addiction, and buprenorphine treatment; screening and assessment of OUDs; protocols for buprenorphine treatment; management of special populations; and policies and procedures related to DATA 2000.
Rates 11 common opiate withdrawal signs or symptoms with a clinician-administered pen-and-paper instrument. Uses the summed score of the 11 items to assess patients’ level of withdrawal and make inferences about their level of physical dependence on opioids.
Facilitates ongoing evaluation of patient pain and documentation of pain management. Includes sections to assess pain-related outcomes in four areas: analgesia, activities of daily living, adverse events, and aberrant drug-related behavior.
Screens individuals for drug dependence, including use of opioids, with an 11-item self-report instrument.
The Alcohol Use Disorders Identification Test (AUDIT-C) is an alcohol screen that can help identify patients who are hazardous drinkers or who may have an active alcohol use disorder.
Consent form on which patient agrees to follow policies related to long-term use of opioids because these drugs have potential for abuse or diversion.
