The Collaborative Care Model (CoCM) has five core principles: Patient-Centered Team Care, Population-Based Care, Measurement-Based Treatment to Target, Evidence-Based Care, and Accountability for Outcomes. CoCM creates opportunities for primary care teams to address behavioral health needs with effective, evidence-based strategies.
Older adults may experience behavioral health needs within the context of multiple chronic conditions, caregiver involvement, cognitive changes, functional limitations, and complex medication regimens. These factors can impact how CoCM is implemented and how teams engage, assess and support patients. This webinar, hosted by the AIMS Center at the University of Washington, will explore adaptations and considerations for delivering CoCM with older adults, with particular attention to workforce preparation, team roles, workflows, engagement, and coordination across medical and behavioral health needs.
