As states and health systems push toward whole person care, integrated models are becoming essential for improving access and strengthening prevention. Join the Primary Care Development Corporation (PCDC) and the Center of Excellence for Integrated Health Solutions for an engaging discussion on how medical, oral, and behavioral health teams collaborate to deliver truly integrated care.

In this session, PCDC Senior Program Manager Shannon Lea will facilitate a conversation with two national leaders in integrated care transformation: Dr. Sue Bornstein, Executive Director of the Texas Medical Home Initiative and co lead of the Texas Primary Care Consortium, and Ankit Sanghavi, Executive Director of the Texas Health Institute. Together, they will explore practical strategies, policy levers, and collaborative approaches that bring medical, oral, and behavioral health providers into a unified system of care.
