NACHC: Value-Based Payment: Move Beyond the Visit – Position for Total Cost of Care (Session 1)

The National Association of Community Health Centers (NACHC) is pleased to announce the launch of a Value-Based Payment (VBP) learning series designed to meet health centers and health center partners, along the VBP readiness continuum – from Planning, to Implementing, to Optimizing.

This 3-part webinar series will guide health center leaders and care teams through practical steps to design and implement a Total Cost of Care (TCOC) operating model that supports population health management, value-based payment readiness, and financial sustainability.

Session 1: Building the Foundation for Total Cost of Care

• Define attributed populations, accountability structures, and units of analysis (PMPM/PMPY)
• Map key cost drivers across the care continuum
• Establish a baseline Total Cost of Care for attributed patients
