Advanced Primary Care Management: Successful Adopters Tell Their Stories

On April 28, join the Primary Care Collaborative (PCC) for a discussion with consumer advocates, policy leaders and early adopters of the Medicare Advanced Primary Care Management (APCM) services about their experience leveraging this new program.  

In 2025, the Centers for Medicare and Medicaid Services (CMS) created a new bundled payment and code set, known as “advanced primary care management.” These codes are bundled to help reduce paperwork and provide primary care with payments on a per beneficiary per month (PBPM) basis.  

These PBPM payments aim to provide primary care with more stability, predictability and flexibility, allowing practices to build out teams and services. In 2026, CMS added additional services — including behavioral integration — and sought to streamline the process even further.
