Practice Transformation: Lessons Learned From Multiple Integration Projects
For their efforts to succeed, practices integrating behavioral health and primary care require transformation on multiple levels. Implementation of elements of the patient-centered medical home model is necessary, along with dealing with culture change and culture clashes. The result is improved quality of and access to health care for patients, but the path to this transformation can be challenging. Dr. Perry Dickinson presented lessons learned from multiple integration projects regarding the practice transformation process.
