Extensible information technology in this context can be used to collect and store expanded data about patients and their environment, especially in assisted living and group home environments. Proposed here is a distributed model for meeting related ONC mandates, which include emerging patient data collection opportunities, especially within nursing homes, assisted living, and other group home arrangements. The conceptual applications employed in this technology set are provided by way of illustration, and may also serve as a transformative model for emerging EMR/EHR requirements.
