Building Health Information Exchanges to Support Accountable Care Organizations and Medical Homes: Delaware’s Experience

In a webinar held last week, the AHRQ Health Care Innovations Exchange and audience members explored ways Health Information Exchanges (HIEs) can promote  Accountable Care Organizations (ACOs) and Patient-Centered Medical Homes (PCMHs). The webinar shared information and lessons learned from the Delaware Health Information Network (DHIN), “an integrated, statewide health data system that [supports] the information needs of consumers, health plans, policymakers, providers, purchasers and research to improve the quality and efficiency of health care services in Delaware.” It also discussed how the DHIN is used for statewide strategic planning, reducing unnecessary medical care and testing, and improving care coordination, as well as key issues about information that supports ACO and PCMH implementation.
