  • You can work through the steps of the Playbook sequentially or customize your approach based on your setting’s Self-Assessment Checklist for Integrating Behavioral Health and Ambulatory Care results.

    • Use the left navigation panel to access a particular section or select “Next Page” at the bottom of the Playbook pages to move through page by page.
    • Tour the Playbook to orient yourself to the Playbook’s layout and features.

    We encourage you to revisit the Playbook as often as needed to refine integration in your setting.

  • Create a free account to take advantage of additional Playbook features, including interactive self-assessments, notes, and the ability to save your progress.

    • To join the Academy, visit the Sign Up page or click the “Sign Up” button on the Playbook Homepage and follow the on-screen instructions.

    • To sign up for the Playbook, you will need to provide an email address and agree to the terms of service.

    If you have previously signed up to use the Academy Community, you are ready to use the Playbook. Simply log-in using your username and password.

  • Bookmarks are only available to registered users of the Playbook.

    A Bookmark is a way to save a link to a specific page within the Playbook.

    • As you move through the Playbook you can set a bookmark to save your place to navigate back to the page later.
    • Once a Bookmark has been set, it will be available by clicking on the “My Bookmark” button located on the Playbook Homepage and under My Account on the left panel on all Playbook pages.

Self-Assessment Checklist Expand AllCollapse All

  • The Self-Assessment Checklist for Integrating Behavioral Health and Ambulatory Care (referred to as Integration Self-Assessment Checklist) is a way to assess integration in your setting.

    • The Integration Self-Assessment Checklist, based on the AHRQ Lexicon for Behavioral Health and Primary Care Integration, is linked to the Playbook so you can customize your implementation approach.
    • The checklist includes 37 questions organized into 11 groups and takes about 10 minutes to complete in one sitting.
    • For each question, click on the response that best describes your situation. You do not have to select a response for each question, although it is encouraged.
    • You can take the self-assessment multiple times before, during, or after implementation of your integrated program.

  • No. Although we encourage you to create an account so you can access additional Playbook features, the Self-Assessment Checklist is available for non-registered users.

    From the Self-Assessment Checklist page, click the “Download” button for an offline version of the Self-Assessment.

  • The Online Self-Assessment is available to registered users of the Playbook. To take the Online Self-Assessment, you must register and log-in to the Playbook.

    • To begin the Self-Assessment, click on the “Take Assessment” button on the Playbook Homepage or click on the “Begin Assessment” button on the Self-Assessment Checklist page.
    • Move between pages of the Self-Assessment using the “Next Page” and “Previous Page” buttons, located below the questions.
    • Once you have finished selecting all of your responses, click the “Finish & View Results” button.

  • Once you have completed a Self-Assessment, click on the “My Results” button located on the Playbook Homepage and on the left panel under My Account.

    You may view prior responses and results each time you update your Self-Assessment.

    • From your Self-Assessment Results page, click on the Results History menu located in the toolbar and select the version you would like to view.

  • Once you have completed a Self-Assessment, you may update your responses from the Playbook Homepage and your Self-Assessment Results page.

    • From the Playbook Homepage, click on the “Update Assessment” button located under Self-Assessment.
    • From your Self-Assessment Results page, click on the “Update Assessment” button located in the toolbar at the top of the page or click on “Update Response” for an individual question.

    Select an updated response to a question(s) and click on the “Save” button. You will be prompted to enter a brief note that will be saved in your Results History menu for future reference.

  • Once your responses have been saved to your Self-Assessment, you can clear your responses if you want to start a fresh Self-Assessment.

    • From your Self-Assessment Results page, click on the “Update Assessment” button located in the toolbar at the top of the page.
    • Then select “clear all of your responses” within the instructions to clear all of your responses.

  • The Playbook provides registered users the ability to create notes linked to pages of the Playbook. Use the notes to record and save thoughts and ideas for future reference.

  • Notes are only available to registered users of the Playbook.

    • To create a note, click on the “New Note” button in the toolbar on any Playbook page. When clicked, a new window will open.

    • Enter a title for the note and use the simple text editor to type your notes.
    • Once you have finished entering text, click on the “Save Note” button.

  • View and edit your notes from the Playbook Homepage, the My Notes page, and on the page that corresponds to your note:

    • The Playbook Homepage provides a listing of your five most recent notes. To view an individual note, click on the “Open Note” button to open the note in a new window.
    • To view a full list of all of your notes, click on the “View All” button on the Playbook Homepage or click on the “My Notes” button located on the left panel under My Account. To view an individual note on the “My Notes “page, click on the “Open Note” button to open the note in a new window.
    • If you are on a Playbook page that contains a note, you can click on the “Notes on Page” menu, located in the toolbar and select the desired note you would like to view. The Note will open in a new window.

    Once a note is open, you may edit your information and click on the “Save Note” button.

    Additionally, you may export and print your notes for reference outside of the Playbook.

  • When viewing a note, you may delete it by clicking on the “Delete” button at the top of the toolbar.

    You can also delete notes from the “My Notes” page by clicking on an individual note’s “Delete” button.

  • Notes may be exported from the “My Notes” page for reference outside of the Playbook.

    • To export your notes, click on the “My Notes” button located on the left panel under My Account.
    • From the “My Notes” page, click on the “Export All Notes” button at the top of the page to export a file containing your notes.
