Building on its existing integrated care and wellness services, Southern California Health and Rehabilitation (SCHARP) will improve health outcomes for adults with SMI and those with co-occurring substance use disorders, primarily minorities who are homeless, veterans recently incarcerated, with trauma history and/or HIVIAIDS in disproportionally affected Los Angeles County (LAC) Department of Public Health Service Planning Area 6 (SPA6), South Central LA.
