The overall goal of the Juneau Primary and Behavioral Health Care Integration Project is to improve the health status of adults with SMI and those who have or are at risk of having co-occurring substance use disorders, who have experienced trauma, and with or at risk for chronic medical conditions. This will be accomplished by integrating primary care, within a behavioral health setting and providing services that are coordinated, cost-effective, person-centered and take a holistic approach to wellness.
