IMC serves adults with serious mental illnesses (SMI) and those with co-occurring substance use disorders who have or are at risk for co-morbid primary care conditions and chronic disease. The service area will be the Brooklyn neighborhoods of Central Brooklyn (including the neighborhoods of Bedford-Stuyvesant, Crown Heights, Prospect Heights, and Brownsville). The service area population is mostly African-American, and suffers from extremely high rates of substance use disorders, mental illness, and chronic primary care health conditions, such as heart disease and hypertension.
